David
you qualifications depend on whether you have formed a company or not

supposing you have formed a company you will have to register as a company first then as the company employee since you and the company are deemed separate legal entities...if it is so then you need a kra pin and certificate of registration
if it is a sole proprietorship then your qualify as an employee and requirements are a copy of your ID card, passport photo and business stamp and name

the scheme is not yet fully operational but in its full implementation you will be able to go to both private and public hospitals with a ceiling put on high cost private hospitals and a civil servant with more than three children
